THREE LAKES WATER ASSOCIATION

As a part of the Cross-Connection Control Program at Three Lakes Water Association we attempt to identify potential hazards due to cross contamination. Part of that program includes the Cross-Connection Control checklist. All new share owners are sent this document in their new share packet. We ask that all members fill out this form and return it to us. If you have not done this please print out the form, fill it out, and mail it to us.

This form helps us to aid all members by ensuring there are no cross-connection issues on their property. Cross-connection issues could lead to water borne health issues on a member's property, or in some instances, affect other members adjacent to such properties.
